        Little Hall, Market Place, Lavenham, Sudbury, Suffolk CO10 9QZ;

I wish to support the work of the Suffolk Preservation Society

Here is my donation of £_____________

Name(s) (Title/Mr./Mrs./Miss) (In block letters)

Address

Postcode

Please make your cheque payable to the Suffolk Preservation Society and return your donation with this completed form.  Individuals who pay income or capital gains tax are encouraged to complete the next section - a gift aid declaration.  This will increase the value of your donation and any subsequent ones, at no cost to yourself.

Gift Aid Declaration

I pay income tax and make the following Gift Aid Declaration, enabling the Suffolk Preservation Society to reclaim tax, equivalent to 29% of my gift.

(Higher rate tax payers can claim further personal tax relief in their self-assessment tax return)

I wish this and all future gifts (including my membership subscription) to the Suffolk Preservation Society (Charity No 249981) to be made by Gift Aid.  If my circumstances change and I no longer pay tax I will inform the SPS.  I currently earn and pay an annual amount of income tax at least equal to the tax that SPS reclaims on my donations in the same tax year
